The qualitative-type questionnaire, with a cafeteria of answers to choose from, it contains questions about key dietary habits and a list of 9 food items, and questions about nutrition knowledge, lifestyle, demographics, socioeconomic and anthropometric data. The reference period for the questionnaire is the last 12 months. The respondents can choose from 7 categories of food consumption frequency: (1) never or almost never, (2) less than once a week, (3) once a week, (4) 2-4 times a week, (5) 5-6 times a week, (6) every day, (7) several times a day.

Based on the data collected, respondents can be ranked in relation to food consumption and it is possible to describe the children's diet quality in general.
